We accept cash, personal checks, and Visa and MasterCard debit and credit cards. 3. A list of current medications. 4. A referral from your primary care physician, if your insurance requires it. 5. Any applicable x-rays (the actual films please) along with applicable reports.

Insurances we accept

We accept almost all major insurances, including, but not limited to Medicare, Advantra and Coventry, Aetna, Humana, United Healthcare and Blue Cross, Blue Shield, and many, many other plans. We are not accepting any new Medicaid patients at this time.

We ask that patients check their individual insurance plans to insure coverage. Patients are also responsible for insuring they have a valid referral or pre-certification for their visits.

Release of Medical Records:

Please find available in this web site a release that you, as a patient, may fill out and bring with you, or send ahead of your appointment, to insure your previous relevant medical records are available for the physician's review prior to your scheduled appointment. Office Hours

Our office hours are 8:30 AM – 4:30 PM, Monday through Friday. Closed Saturday, Sunday and all major holidays, but one of our physicians is always on call, 24 hours a day, 7 days a week. This on-call physician is accessible by calling our general phone number and following the prompts for after-hours emergencies.
